Ordering for any healthcare organization can get complicated. Here is a list of everything we’ll need from you. Please email the documents below to contact@patientcareboards.com along with any requests you have for us.

Print PDF Checklist

  • Your logo in .eps format

  • Your organization’s brand colors in PMS (Pantone Matching System)

  • Any specific messaging needs for your inserts (e.g. CALL DON’T FALL, EXCELLENCE IN CARE)

  • Sends us a sketch, a Word.doc or PDF of your ideas. We’ve designed inserts that were written on napkins! Don’t worry about the specific design of your insert – leave that to us – just concentrate on the information you want to have included on your insert.

  • Do you need separate inserts for non-English speakers? Do you want to incorporate language alternatives into your main insert? Let us know!

  • Please provide your billing information, including address with zip code.

  • We’ll need your organization’s W-9, as well as any other tax-exempt forms you may have.
